Actual resident here. To start all these five star reviews are put on here by employees pushed to do so by administration. Second. The place is ran right now by a dude younger than me. Im 30. One look at his linked in profile showes hes been at 4 different places like this and each time was there less than a year. He brags about cutting costs and his main focus is getting people here who require the least care and who lack the ability to speak up for themselves. This place focuses on trying to run like a old folks home where people come to die instead of heal. The fact they look for hospice patients specifically is terrible as I can say this is the last place id want to pass away in. 3 beds to a room and two rooms share one tiny bathroom thats rarely cleaned thoroughly and consistently has leaking toilets causing fall hazards. Social services here is a joke. For 6 months ive asked for help preparing to transition and in that time theyve gone through two employee’s for that department which is ran by one person for an entire building as is. Ive asked for detailed incident reports several times and been refused eachtime. They lack the ability to even obtain my medical records per requested so I can finish up my social security disability application. Ive been given a months notice to get out or I will be charged 550 a day because my insurance dropped me less than a month after receiving major knee surgery which is also my 5th surgery in a year and thats because this place purposely had the state show up at theyre request for my evaluation and was literally the day I wasnt here conveniently and had the surgery I mentioned and pushed that im ready to leave through purposeful omission of key details of my injury and situation. They’ve allowed drugs to run rampant through the place focusing on things to the point theyve had a lady out back sleeping on the patio because she was having an episode and relapsed and refused to contact other options for unknown reasons. If you speak up for yourself they will HARASSS YOU. DO NOT BRING PEOPLE YOU LOVE HERE. I already know woodside will retaliate for this post but idc sincerely a resident thats 30 years old and clear of mind able to speak up unlike half the other residents here. Anybody with any questions can message me with questions for any amount of time this is up. If you got family here check in more and start reporting. This place should be shut down. They just had theyre evaluation and it for two weeks it ran close to how it should be until they where gone and then went right back to being crap. They also have a weird habit of trying to cut off pain medication and put everyone on suboxone. Reference posted pictures i added and ask yourself why hes had to switch states every year after only ever making it past a year once at his precious appointments. Dude clearly also didnt pick this as his first career choice either. Wheres the passion seth? Instead you lurke around corners spying on employees and other and seem more focuses on your outfit for the day than actually bring about better medical outcomes.

Staff is very friendly and knowledgeable. The residents make this one of the best communities I’ve had the pleasure of visiting.

DANGEROUS!! NOT SAFE! NOT SAFE! DO NOT TAKE FAMILY HERE!!! My grandmother, who is recovering from brain surgery and has clear cognitive and memory problems, was placed here a few days ago. Today, they allowed her to walk out of the facility unaccompanied and pace and wander in the streets until the police showed up and took her to Kaiser. They are not equipped for such care and should not have taken her as a patient from Providence, where she was transferred from. They put my grandmother in physical danger because of their lack of adequate care for her needs. I will be contacting the state and reporting this facility and Providence to the ombudsman, APS, and the state licensing board.

Absolutely the worst rehab center! We have a family member there preparing to be discharged today and they have lost all his clothes – literally – and have him in someone else’s clothes. They are doing nothing to remedy the situation except “offering to order stuff off of amazon” after we supply a wish list. Are you kidding??? He is being discharged today with nothing. They have also lost his walker – which he is supposed to be using daily to do therapy. So has his therapy been happening? No one can tell us. Worst communication, even worse patient care. Filthy. Do yourself a favor and avoid this place. No answers, no calls, and no one responds. If I could give it negative stars, I would!

Caring staff that treat residents and families with dignity and respect.

I have lived and worked in the United States for many years, and this was the first time I experienced being treated this way in a workplace. In my experience, the HR staff did not treat me with professionalism or respect. Instead of respectfully asking me to come to the office to discuss concerns, I was unexpectedly approached while I was working. The way the situation was handled made me feel intimidated and disrespected. I believe every employee deserves to be treated with dignity, professionalism, and respect. I hope the facility improves the way it communicates with and supports its employees.

Went to see my sister, her room smells like pee. Asked for a pain pill for her, and when I was leaving they was standing at the station talking. My sister had oxygen on that was not putting out air.

Everyone. There is so nice and helpful if you need anything. You just ask anybody, and if they can’t do it, they will find someone who will, they’re very eager to help. And like I said, very nice, and I don’t know of a better place, my wife could be at this point very happy with everything.

We went in to see a friend who is in rehabilitation. When we went to the desk the nurse really told us, “I dont know who that is or what he looks like”.. and then I saw why. They have the beds 3 deep in the rooms, with one bathroom per room. The nurses do not answer the nurse call button, when the button decides to work. The social worker does not come around to talk to the patients about what is going on with their case or to let them know what the next step is in their care plan. When we went to leave we couldnt figure out the eay to get out of the building, because the door wouldnt budge. I went and asked for help and got met with an attitude for asking for help. I would pass this place up. Definitely do not put anyone you care about here.

The staff that took care of my mom was beyond awesome. Especially during the time of passing. I would give 5 stars but on of the nurses was inappropriate with me and they only wrote him up. I file be filing complaint to the board of nuring and any where else i can But hank u all tge others who took care of mom

The staff is plentiful and very accommodating. The facility is clean, well-maintained, and creates a warm, welcoming environment for both residents and their families. From rehabilitation services to long-term care, every aspect is handled with professionalism and genuine kindness. Highly recommend for anyone seeking exceptional skilled nursing care.

not good , they go through the motions and dont care. they dont follow through with support and dont send the right equipment home when discharged or give patients the correct meds.

Recently my mother was in Wood village for rehab for 20 days. Not one time was the staff ever helpful or even nice. They acted like they didn’t have the time to deal with myself or my mother. But the big issue was when it was time for her to be discharged they did so without informing anyone dumped her at a hotel with no money and no phone. Then told me it was my problem and that they left her a bag of food they then hung up on me and I felt as if it was an emergency and requested the administrator contact me back but ONCE AGAIN NO RESPONSE FROM THEM. my mother isn’t suppose to be left alone and they found it completely acceptable that they dropped her off at 7 pm at night at a hotel she couldn’t even check in to so please please don’t use this facility. They also dropped her off with a bill for ,[price removed] cause they didn’t catch it had been 21 days. A day over insurance payment.

Horrible horrible service. I would not recommend anyone place a family member at this facility. It’s packed staff is rude and I mean rude. They discharged my disabled mother without informing anyone. She was left at a hotel that they claimed to have paid for for her for a few nights due to her home not being safe. She had no money no id to even check in to a room. She isn’t suppose to be left alone and it was 7 pm to begin with. I am so disappointed in how they handled it when I found out that she had been dumped off I contacted the facility and they told me it was my problem and there was nothing they could do about it. Then hung up on me. I called back asked the administrator to contact me as I felt this was an emergency and again hung up on by the charge nurse who was rude and never heard from the administrator. It makes me sad that company’s would treat a disabled woman this was. My opinion it should be abuse or neglect. The 20 days she stayed there not one time did I have a pleasant or nice interaction with any staff. SO PLEASE PLEASE DONT PUT YOUR FAMILY HERE. they also sent her with a bill for 550.00 cause they said she had 20 days to stay but they didn’t notice it had been 21 days. Just craziness.

The staff is plentiful and very accommodating. They went out of their way to set up lunch for my friend on the outside patio. Unfortunately, message didn’t get through and her lunch was delivered to her room. The staff is very friendly. The building and rooms are crowded and not very clean.

The staff are great. We were there to meet with a nurse from Serenity and the staff was kind enough to offer the use of their conference room.

Please do not send your family members here or a loves ones they neglected My uncle , who has a trach and cannot speak for himself only way of communication is reading lips and him writing on a whiteboard and they did not provide a whiteboard for him. They fed him once at 6 AM and did not feed him or give him any pain management meds when he just had surgery less then a week , removing his voice box and have staples all around his throat/neck and on his side and is in severe pain and has asked for pain medicine and did not receive when I was there at 9:30 AM. I asked for pain medicine for him. The nurse provided Tylenol only and said she would look at his chart and see what else she could give her never provided anything I left around 10:30 am and received a call at 7pm saying my uncle wanted me to come talk to him I got to him around 745pm and he told me that he has not ate or had any pain management. One of the nurses tried to inform me that she offered him a tray, but he is Nothing by mouth as someone that works in healthcare this should never happen. No one should ever be treated this way and I want it to be known that this place does not treat our loved ones the way that they should.

The staff are kind and obviously care about making a difference for the patient.

Mistreats their patients, they order from a menu but never get what they ordered and the food is horrible!! I really wish my loved one did NOT have to be there!! People need to realize that hopefully they will get old too. I hope they are treated the same way they treat their patients now!! Makes me sick!!!

From the moment we met her, she showed nothing but genuine care, professionalism, and compassion for both our loved one and our family. Jessica goes above and beyond and takes the time to listen, answer questions thoroughly, and does so with clarity and empathy. Her calm, reassuring presence made a stressful time feel manageable—and even hopeful. She truly advocates for her patients and brings a level of warmth and dedication that makes all the difference. We are so grateful for everything she’s done and continue to feel lucky knowing she’s a part of the team.

I can’t speak highly enough about this 5-star skilled nursing facility. The staff are compassionate, attentive, and consistently go above and beyond to ensure residents receive the highest level of care. I’m a pastor and was visiting a member of my church today. I watched as the respiratory therapist gave her a treatment – wonderful personality, great sense of humor. Other staff seemed so friendly as well. And my church member is so much healthier than when I last saw her in the hospital. Keep up the great work! You’re a blessing!
